Medical Forms

Before the start of the first sport you are playing for the year, you must upload the PIAA CIPPE (Medical Examination Forms) to Arbiter. If you play another sport during the school year, you will only need to complete the PIAA Recertification Form.

Physical forms for the 2026-2027 school year have to be completed after May 1st, 2026, to be valid for the 2026-2027 school year.
